Activity of cefepime/zidebactam (WCK 5222) against ‘problem’ antibiotic-resistant Gram-negative bacteria sent to a national reference laboratory

Abstract Background Triple-action diazabicyclooctanes, e.g. zidebactam, combine ?-lactamase inhibition, antibacterial activity, and ‘enhancement’ of PBP3-targeted ?-lactams. Objectives To examine the activity cefepime/zidebactam against consecutive ‘problem’ Gram-negative bacteria referred to UK national reference laboratory. Methods MICs were determined by BSAC agar dilution for 1632 Enterobacterales, 745 Pseudomonas aeruginosa 450 other non-fermenters, categorized carbapenemase detection interpretive reading. Results Universal susceptibility 8 + mg/L was seen otherwise multidrug-resistant Enterobacterales with AmpC, extended-spectrum, K1, KPC OXA-48-like ?-lactamases, or impermeability ‘unassigned’ mechanisms. Unlike ceftazidime/avibactam all comparators, also inhibited most (190/210, 90.5%) MBLs. Resistance in remaining minority MBL producers, 13/24 both NDM MBLs enzymes, associated Klebsiella pneumoniae ST14. For aeruginosa, rose efflux grade, but exceeded only 11/85 isolates even highly-raised group. Among 103 P. ESBLs MBLs, 97 (94.5%) whereas fewer than 15% susceptible any comparator. Acinetobacter baumannii acquired OXA carbapenemases clustered around 32 mg/L, higher values producers. A strong enhancer effect augmented many that highly resistant cefepime zidebactam alone which had mechanisms not zidebactam. Conclusions Assuming successful clinical trials, has scope widely overcome critical resistances non-fermenters.
